Sometimes, patients who are on hospice experience stabilization of their disease to the point that they no longer meet the Medicare requirements for hospice. Being discharged from hospice can be overwhelming to patients and their family care partners who are faced with continuing care needs.
Starr Hospice designed the Advanced Illness Care (AIC) program to ease the transition from hospice by continuing to provide guidance on care solutions and to connect the family with needed resources.
Our AIC program is currently available only to individuals who are being discharged from our hospice services. Members of the Starr Hospice’s AIC team work together to provide support in the following ways.
AIC families are also welcome to attend Starr's education series that provides resources and support to caregivers of family members with life-limiting or disabling conditions. Learn more about the Family Caregiver Education Series, which is provided at no cost to both our client families and the public in our service area. Several classes are dedicated to dementia-related care.
